Important Update Regarding UnitedHealthcare (UHC) Coverage

As of October 1, 2025, Madison Medical Affiliates is considered out-of-network with all UnitedHealthcare insurance plans. This change follows the expiration of the contract between Ascension WI & UHC.

We understand how concerning and frustrating this news may be. UHC covers many of our patients and their families, including our own staff. Please know that this development does not change our commitment to providing you with compassionate, high-quality care.

At this time, patients with UHC may experience higher out-of-pocket costs when receiving care at Madison Medical. We encourage you to:

  • Review your insurance benefits to understand how out-of-network coverage works under your specific plan.
  • Visit UnitedHealthcare’s website or call the customer service number on your insurance card for more information about your options.
  • Reach out to our office with questions - we will do our best to guide you and provide support.

While Ascension WI & UHC may continue discussions, we must move forward under this out-of-network status until further notice. We will keep you updated on any changes as soon as we learn more.

Office Policies

Thank you for choosing Madison Medical Affiliates in Milwaukee and Mequon, WI for your healthcare needs. We are proud to serve you and our community. Our goal is to provide you with the highest quality care through experienced medical physicians and dedicated primary care providers. To ensure the highest quality care and to prepare for your upcoming visit, please review our office policies below.

In preparation for your appointment, please plan to arrive 15 minutes before your appointment time to register and bring:

  • Insurance card
  • Photo ID
  • Cash/check/credit card if copay required
  • Up-to-date medication list

Office Policies

Cell phone policy

When entering our office please put your phone on silent/vibrate but feel free to use your cell phone within our waiting rooms. After being called back for your appointment please refrain from using your cell phone until your appointment is finished.

Copays/Prior Balances

Copays and payment on prior balances are expected at time of service, unless a prior arrangement has been made with our billing department.

No Show

We value the time of our patients and providers and understand that appointments may need to be changed. We ask that  you provide 24 hour  notification to cancel or reschedule your appointment. Please see policy below if ‘no show’ or cancelling within less than 24 hours.

New patients: Two no show appointments will result in dismissal from the practice

Established patients: Three no show appointments will result in dismissal from the practice


Billing/Insurance Concerns

Please keep in mind that every insurance company is different and some do not cover certain services. We encourage all patients to contact their insurance company prior to services rendered. If your physician can do same day treatment for your convenience in one office visit, your insurance may not cover that treatment and you may be charged the full balance.
