What To Bring
In preparation for your appointment, please plan to arrive 15 minutes before your appointment time to register and bring:

- Photo ID
- Cash/check/credit card if copay required

Office Policies
Cell phone policy
When entering our office please put your phone on silent/vibrate but feel free to use your cell phone within our waiting rooms. After being called back for your appointment please refrain from using your cell phone until your appointment is finished.
Copays/Prior Balances
Copays and payment on prior balances are expected at time of service, unless a prior arrangement has been made with our billing department.
No Show
We value the time of our patients and providers and understand that appointments may need to be changed. We ask that you provide 24 hour notification to cancel or reschedule your appointment.
